Acceptable Supporting Documentation

A Note on member PII/PHI:

Please do NOT put member(s)' name(s) in the file name for any uploaded documents. Instead list them by what they are for. 

For example: put "Lease agreement" instead of "John Smith" or "JS Lease"

 

Must have a fully filled out and legible W9 for Fund Recipients:

  • W9 must have Line 1 filled out
    • The name in Line must be what is input in the payment request as the Fund Recipient name
    • If the check needs tb paid to a different name, please note this in the address or the Other option under what the funds are for
  • Address
    • This does not have to match the check delivery address
  • EIN/TIN or SSN
  • Signed and dated

 

Supporting documents must have:

  • Must connect Member to the fund recipient
  • Must show total funds being requested
    • If breakdown is not explicitly stated in supporting document, please select Other and provide a breakdown in the provided textbox
      • Example: If the total requested amount for a rental payment is $1,100 but this includes both a security deposit of $600 and 1st month rent of $500 then this would be explained in the Other section.
  • Note- if any additional information is needed to complete payment it needs to be in the supporting document or provided in the Other textbox

Examples of acceptable documents for requests:

Housing-related (i.e., application fee, security deposit, rental assistance, rental arrears) a lease document or letter of intent to rent from the landlord is required. The information that must be provided within either document is:

  • Member’s name
    • If the name does not match the lease and no additional documents to support this are provided, then LSS must contact CC for further documentation. Documents that can be provided include a marriage certificate or name change documentation.
  • If a member is a minor, then they must be named in the supporting document with the same first and last name as listed in the request.
    • If a minor is not listed on the lease or letter of intent, supporting documents include a birth certificate supporting the relationship to the adult member.
      • Can be used if the minor’s last name does not match parent/adult.
    • The recipient’s name
    • Amount requested

 

Hotel stays the supporting documents must be a receipt or invoice statement. The information that must be provided within the receipt is:

  • Member’s name
  • The recipient’s name
  • Amount requested

 

Utilities (i.e., utility deposit, utility arrears, utility payment) the supporting document must be the member’s account statement:

  • Member’s name
  • Amount requested

 

Other notes on supporting documents:

  • The file type for all documents must be a viewable document that does not require downloading PII so they should be uploaded as jpeg, PDF, or excel file. PNG files are not viewable and not accepted.
  • Recipient’s W9 should be uploaded separately from the member’s supporting document and a file named as recipient's name.
